Case Manager
Position Summary: Case Manager will be responsible for assessing, identifying needs and appropriate community resources, planning, offering choices of service providers, arranging needed services, monitoring the provision of services, providing immediate crisis management services as deemed necessary, arranging needed services, monitoring the provision of services, providing crisis assessment, conducting referral services and needed follow-up, communicating with other involved agencies and/or providers, assisting and coordinating services needed the most for persons affected by autism spectrum disorder (ASD) and their family members.
